Rehab Jigsaw is able to accept both single and joint instructions from solicitors and insurers and can undertake one-off assessments or have longer-term case management involvement.
Rehab Jigsaw OTs recognise that Rehabilitation Case management (CM) is a collaborative process of assessment, planning, facilitation, care coordination, evaluation, and advocacy. Case management provides options and services to meet an individual’s and family’s comprehensive health needs through communication and available resources to promote quality, cost-effective outcomes.
Working together for the good of each client.
Throughout the Case Management process, we work holistically with a client and their families to achieve their goals. Our OTs are dual trained in both physical and psychological medicine. Our work is client-centred and goal orientated. We assess all of the needs of our clients, including physical, psychological, social and environmental. We make recommendations to assist adults and children back towards a better quality of life. We enable a greater participation in home, leisure, school and vocational activities and understand the impact illness and injury can have on those around you.
Our support can make a real difference to overcoming the effects of disability, regardless of their cause, to enable people to carry out everyday tasks or occupations.
